Electrocardiogram
This is a task that you will do with the help of Dr. Hobar and his staff. Once you schedule your surgery, Dr. Hobar’s staff will contact you to discuss any preoperative work that you might need to do. This could include lab work, an EKG, a mammogram or preoperative clearance from another physician such as your internist, a cardiologist or other physician specialist. These preoperative measures are in place to make sure that you are healthy and ready for surgery. Dr. Hobar and his staff take great pride in providing excellent patient care and the most critical component of this care is your safety and well being.Independently, make sure that you are generally healthy and well rested before your surgery. Make an effort to eat well and get plenty of sleep during this time. Carefully read your surgical packet and review the list of medications and supplements that you must avoid.

Brow Lift
The cosmetic surgical procedure known as a brow lift is preformed under general anesthesia and addresses aging, excess skin and laxity in muscles and connective tissues in the upper face region that is comprised of your forehead and eyebrows. According to the American Society of Plastic Surgeons, in 2014, over 40,000 patients in the United States had a brow lift done.By using a simple principle to combat the potential unnatural results that some cosmetic surgeons provide, Dr. Hobar’s approach to a brow lift is bidirectional. As he states, “The key to achieving a natural, long-lasting result with a brow lift is to address tissue migration in two directions. It is a mistake to only raise and support the tissue vertically, this is oftentimes the technique used when you see people with a perpetual surprised or wide-eyed appearance. The additional support must also extend to an acute, outward angle, bisecting the temple region. When the same amount of support is provided in this bidirectional nature, the result is incredibly natural. The patient benefits from a more rested, alert and pleasant expression. In addition, aging along the sides of the eyes commonly referred to as “crow’s feet” can be subtly softened.”
Eyelid Surgery
At some point in their lives, either very early on in the aging process or in the latter part, women and men in Dallas generally fall into one of three categories. The first group includes those that would never consider having their eyes done because they hate the prospect of seeming vain or are frightened at the possibility of having an alarmed, wide-eyed unnatural look – or worse, one that is not healthy because too much skin has been removed from the eyelids and they cannot properly close and do their job of lubricating the eyes. The second group includes those that have unfortunately had surgery with less than desirable outcomes. The third group – and the one you want to be in – includes those that have had their eyes done by a well qualified plastic surgeon that created a very natural, beautiful result. Excess skin in the upper eyelid can be aesthetically unappealing, but can be obtrusive as well. It can make reading difficult and cause your eye makeup to smear. Aging in your lower eyelids will literally stop people in their tracks to ask you, “Is everything OK? You look so tired!” And you are not tired, but you are tired of being asked that question. Eyelid Surgery is the second most common cosmetic surgical procedure in the United States. According to a survey by the American Society of Plastic Surgeons (ASPS), in 2014, over 200,00 patients in the United States had this procedure performed.
Liposuction
A third group that benefits from chin surgery are individuals with a genetic predisposition to full necks, creating the appearance of a “double chin”. While it is possible that this can be solved with liposuction or skin tightening, oftentimes adjusting the shape, projection or length of the chin can be efficacious alone.
Breast Augmentation
This is a question that everyone who considers breast augmentation must at least deliberate. Unfortunately, many women come into Dr. Hobar’s office with a great deal of misinformation that they have acquired over the years simply by being consumers that listen to marketing paid for by implant manufacturers and from listening to urban myths about the different qualities of saline vs. silicone implants. Rhinoplasty
Patients presenting for rhinoplasty with a nose that is “too long” for their face – Nothing makes a nose look too long for a face more than a small chin. Dr. Hobar has published work on measurements of facial proportions in faces chosen for their attractiveness (models and beauty contestants) based on a large study he completed while in training in late 1980s. Dr. Hobar will do a complete facial analysis with you and discuss the balance of your facial features. If he recommends changing the dimensions of either the nose or the chin he will personally do a computer imaging of the changes he recommends.
Breast Reconstruction
“I struggled with what kind of medicine I wanted to dedicate my life to when I was in medical school. I knew it was some form of surgery, and I thought it was probably heart surgery. I was fortunate enough to be given the opportunity to do an externship in Dallas, the program that I had heard from my guidance counselor was the best surgical training program in the country. But, heart surgery wasn’t available, only plastic surgery. I accepted, but was initially disappointed because of my preconceived notions. I was absolutely blown away when I started and found world class surgeons doing procedures that were on the forefront of surgery. It was the beginning of breast reconstruction for women after mastectomy. The surgeons were taking tissue from the back and tummy and creating beautiful breasts that gave these women hope for the first time that they could not only be cured from their cancer, but again have a normal, beautiful figure. In another room, there was a microsurgeon meticulously working under a microscope to replace a hand that had been severed in an accident. In still another room, a surgeon was reassembling a young child’s face who had been born with a severe facial disfigurement. When I saw the passion in these surgeons and the joy in the patients, I knew, right then and there, what I wanted to spend the rest of my life doing.”

Face Lift
Dr. Hobar is a board-certified cosmetic surgeon who is very well known for creating natural, long-lasting results with his facelift surgery. He has been in private practice since 1990 and brings a unique approach to the playing field having developed safe and effective procedures that produce long lasting and powerful results while always maintaining a totally natural look. An ideal candidate is a patient who looks in the mirror and says, “When did I begin to look different than I feel. I certainly don’t feel as old as I look”.
Neck Lift
A minimal amount of skin looseness can sometimes be improved with the above procedures, but anything more than this requires surgical tightening. The way to tighten the neck skin with the most natural result and least visible scarring is through a lower face and neck lift where incisions are placed in the crease around and behind the ear. Like in facial rejuvenation, the only way to get a natural and long lasting result is to tighten the muscles as the central support of the skin, so therefore, platysmaplasty (or surgical tightening of the neck muscles) should always be a part of a neck lift.
Blepharoplasty
However, there are plenty of patients who have aging and changes in the eyelids that are out of proportion with the other regions of the face. These patients are ideal candidates for having a blepharoplasty done in isolation.
Nose Surgery
Dallas and Fort Worth rhinoplasty patients should know that the nose is one of the defining structures of the face. If it has a nice shape and is of proper proportions, it brings into focus and compliments the remainder of the face. If it has an undesirable shape, is too long or too short, then it is difficult to get past the distractions of the nose to other facial features. Reshaping the nose, or rhinoplasty, is one of the most common cosmetic surgeries performed. The American Society of Plastic Surgeons (ASPS) reports that nearly 220,000 rhinoplasties were performed in 2014.The Dallas, Fort Worth (Metroplex) has evolved into a wonderfully diverse mixture of cultures over the past decade. Future projections promise further diversification. With this change, more demand has arisen for plastic surgeons to be sensitive to ethnic variations among their patients’ facial features. Rarely is this diversity manifested as universally as it is in the nose. Rather than seeking a bland, neutral nose that is attractive but lacks individuality and character, patients are coming to Dr. Hobar with the interest of improving aspects of their nose while not altering the defining features that add character and grace to their overall facial symmetry. In addition to being a board certified Dallas plastic surgeon in private practice for over 25 years, Dr. Hobar specializes in rhinoplasties. He strives to avoid the operated look when performing rhinoplasty. His sensitivity to each of his patient’s unique features and desires is compassionately expressed in his personal view of the desired outcome of nose surgery.

Otoplasty
Otoplasty, commonly known as ear pinning, is a surgical procedure that corrects the floppy or protruding ear. It is a beautifully artistic procedure that uses a hidden incision behind the ear. The cartilage is carefully shaped to highlight the three dimensional architecture of a normal ear.
Lip Augmentation
Dr. Hobar prefers to use Alloderm (collagen or protein tissue like substance derived from skin) when he performs lip augmentation as a surgical procedure. Alloderm always gives good short term augmentation, and has a chance of giving good long term augmentation. Alloderm use in lip augmentation is off label, meaning that it has been thoroughly studied and approved for use as a graft material, but not specifically for lips. It would be impossible to think that once a material or drug is approved, the laborious and extremely expensive process of re-approving it could be done for every single use. Many materials and drugs are used off label because it is the practical approach, once a drug or material has been approved safe and effective. That doesn’t mean you shouldn’t understand the material and drug and the reasons it is being used off label. You should question the doctor and insist on a thorough understanding. Alloderm has been used in numerous patients for lip augmentation for years and has an extremely high safety record. The main drawback with Alloderm is that it doesn’t always last. The only thing that can guaranty a long term preservation of volume is an artificial implant such as silicone or gortex. Dr. Hobar is very opposed to these type of implants.
Body Contouring
It is impossible to enjoy the long term benefits of body contouring surgery without a proper balance of diet and exercise. One does not replace the other, one compliments the other. We are all genetically and hormonally prone to store fat in certain areas. Even when one is at an ideal body weight, there are usually areas still harboring fatty pockets that are out of balance with the rest of the figure – these are ideal sites for liposuction. One does not have to be at his or her ideal weight to undergo liposuction, but should have adjusted their lifestyle to where he or she can continue to gradually lose excess weight and even more importantly, not gain weight back. Anyone can overcome the benefits of liposuction with rapid and massive weight gain. If one is on a nutritional and fitness program where his or her fitness level and weight continue to improve, the benefits of liposuction will last indefinitely. Many patients initiate a fitness and nutritional lifestyle change with the goal of undergoing surgery. The body contouring surgery can provide incentive for them to work hard before the surgery and provide incentive for them to continue after the surgery to maintain and even improve on the results.
Cosmetic Surgery
Most people will have benefits from photofacial treatment. Patients who have sun damaged skin, patients with redness, vessels, dark age spots, younger patients who want to prevent aging skin, middle-aged patients who want to reverse signs of aging, post-cosmetic surgery and laser patients, and all skin types I-VI. Please discuss all medical history with your provider prior to scheduling your treatment. It is vital to avoid recent sun exposure, light-sensitizing medications, and Accutane. We ask that you stop all retinol containing products 1 week prior to your procedure. Let our office know if you have a history of herpes simplex (cold sores), history of sensitivity to lasers, history of adverse reactions to prior treatments, and any recent cosmetic treatments.
Dermabrasion
Facial Implants
People with strong facial bone structure age more gracefully than those with weaker bone structure. There are many classic consequences of inadequate bone structure. The negative vector is a term given to individuals with weak support of their midface region. This results in premature aging of the lower eyelids and midface and presents a difficult challenge for the cosmetic surgeon. Patients with small chins are predisposed to accelerated aging in their lower face and neck regions. Patients with weak cheekbones are prone to premature aging in their midface, lower eyelids, nasolabial folds and jowl areas. Not only are patients with weak bone structure prone to premature aging appearances, but they frequently lack the definition and character that is associated with beauty. Think of the high cheekbones many of the beautiful actresses, or the striking, defined lower jawline that characterizes the faces of models that frequent fashion magazines. Dr. Hobar has a special interest in evaluating bone structure when he is considering facial rejuvenation procedures in a patient.

Since I became a plastic surgeon twenty four years ago, the field of skin rejuvenation has changed radically. It has grown from a position where the large market was dominated by cosmetic companies selling their products in drugstores and department stores to one that has been greatly influenced by scientific advances in the dermatology, biochemistry and plastic surgery. There are products today that if used appropriately and consistently can greatly improve the texture and color of skin as it changes over time and from external forces such as the environment and lifestyle choices.The revolution that injectables has brought to plastic surgery in general has benefited my patients. We are able to use products such as Botox®, Juvederm® Ultra Plus, Juvederm® Volume XC and Belotero Balance® to offer patients beautiful, natural results that enhance their image, self confidence and subsequently their quality of life. While these options do not enact permanent results, they are available to a larger group of people because they are an economical option – in terms of dollars and recovery time – in contrast to cosmetic surgery. Frequently they allow patients to wait longer before undergoing a major facial rejuvenation surgery and they have the added bonus of enabling people to get a sense for how much they would enjoy the results of cosmetic surgery.
Acne Treatment
Eczema
Generally, TCA peels are preferred for lighter skinned individuals to prevent hyperpigmentation. Patients with darker skin may be excellent candidates for different peels, it is important to discuss options with a qualified provider. Patients should inform the office if they have had recent sun exposure or a current sunburn. It is important to discuss any medications you may be on, as some increase your skin’s sensitivity. Chemical peels are not recommended for women who are pregnant, nursing, or patients who may have taken Accutane in the last 6 months. Please inform your provider if you have a history of cold sores, as chemical peels can trigger these. Patients with rosacea, facial eczema, or psoriasis should take extra caution as these can be exaggerated after a peel.
Chemical Peels
Dermaplaning is a way to remove excess dead skin cells and peach fuzz to leave a smooth texture to your skin. Removing the top layer of the skin allows other products to penetrate deeper and allows makeup to be applied more flawlessly. Often times, dermaplaning can be used prior to a laser treatment or a chemical peel in order to achieve a greater result.
Skin Care
The Fraxel laser uses points of light to precisely treat thousands of microscopic areas of your skin. This treatment penetrates deep into the skin to remove old, damaged skin cells, which leaves your skin with a healthy, youthful glow. We have found that the Fraxel Re:Pair® Treatment has been very effective in tightening skin, softening deep wrinkles, all while dramatically improving the texture and appearance of your skin. This procedure takes approximately 15 minutes to one hour depending on if you are having the laser around your mouth, or your full face.Typically, Dr. Hobar performs Fraxel Repair Treatment in a operating room setting. This can be done as a stand-alone procedure or may be done in conjunction with other facial procedures, such as, facial rejuvenation, brow lift or eyelid lift. Fraxel laser treatment can be repeated if necessary; however, you may find that one treatment will produce the results you have been looking for. Following this procedure you may expect to have reddened skin tone – much like a sunburn – anywhere from 24 hours to one week after you have Fraxel laser treatment. You will be placed on a skin care regimen two weeks prior and two weeks following your treatment, and should be back to wearing makeup within five days following your procedure. Our patients have stated this procedure was painless, and experienced very little down time. We have seen very good results, and patients have expressed significant happiness with Fraxel Re:Pair® Treatment.
Psoriasis
Jessner peels have been found to improve texture and coloration for both lighter and darker complexions. Patients should make sure to avoid sun exposure before and after the chemical peel. Many opt to undergo this procedure during the fall and winter months when they are less likely to be outside. It is important to discuss any medications you may be on, as some increase your skin’s sensitivity. Chemical peels are not recommended for women who are pregnant, nursing, or patients who may have taken Accutane in the last 6 months. Please inform your provider if you have a history of cold sores, as chemical peels can trigger these.
