Endometriosis can spread to a variety of locations. It is common for patients to be unsatisfied with their endometriosis surgery when surgeons ignore their bowel symptoms and do not recognize their bowel lesions during surgery. Patients often receive a misdiagnosis of irritable bowel syndrome (IBS) or Crohn’s disease, as physicians mistakenly attribute their symptoms to a gastrointestinal cause. It is important to ask whether or not a patient’s bowel symptoms are in sync with their menstrual cycle. If doctors do not ask these questions and do not alert the surgery team beforehand, the lesions will go unnoticed, unremoved, and untreated.

Female Infertility
Endometriosis materializes when the endometrium – the tissue that lines the inside of the uterus – sheds but does not properly exit the body during a woman’s period. Instead, this tissue grows outside of the uterus, spreading to organs and nerves in and around the pelvic region. The resulting pain is so physically and emotionally insufferable that it can mercilessly dominate a woman’s life. The average woman with endometriosis is twenty-seven years old before she is diagnosed. Endometriosis is one of the top three causes of female infertility. The pain can affect a woman’s career, social life, relationships, sexual activity, sleep, and diet. It is incurable but highly treatable. Unfortunately, the disease is rarely treated in a timely manner, if at all, because of misdiagnoses and/or a lack of education among those in the medical community.

I moved to New Haven and started having symptoms worsening. I was getting a cyclic monthly period, which was of course painful, despite my religious use of continuous Necon. Dr. Laufer recommended Dr. Hugh Taylor at Yale. Dr. Taylor did an ultrasound to make sure there wasn’t anything huge Dr. Laufer missed, and he couldn’t see any endo. He suggested Orilissa, which I agreed to fill with counsel from my orthopedist who had recently performed a surgery that involved breaking my femur, which hadn’t healed as well as they’d hoped. I held off, and my orthopedist said no way. I began taking medroxyprogesterone orally in an attempt to try anything else, despite knowing the norethindrone I took in high school caused a 35-pound weight gain. As expected, medroxyprogesterone caused a 15-pound weight gain in 6 months, but my pain was improved. We knew this wouldn’t control my disease progression though, due to my history, so as soon as my orthopedist cleared me, I began Orilissa 200mg 2x/day + medroxyprogesterone addback. When I went in for a follow-up, we agreed to lower my Orilissa dose to 150mg daily to eliminate the need for addback, hopefully combating my weight gain from medroxyprogesterone. I had a Mirena IUD inserted too since I was left without birth control. Dr. Taylor did an ultrasound a month after that to make sure it was in place, and looked around the whole pelvis and said it looked good.
MRI
An MRI allows a physician to see all the glands of the myometrium, the thickness of the junctional zone (which will be thicker than 1cm in case of adenomyosis), and asymmetric walls. An MRI allows your doctor to see whether or not you have adenomyosis first hand, with a sensitivity rate of 88% and a specificity rate of 93%.
Computed Tomography
Imaging tests: Additional tests like an MRI and a dual contrast CT scan are necessary to evaluate the condition of the ureters and higher bowels, and appendix. The appendix is one of the first organs that, bowel endometriosis infiltrates. For this reason, it can be a key finding in presumptively diagnosing endometriosis before surgery.

Neurosurgery
I was kept on Junel for about a year, but it wasn’t really helping my pain and naproxen 500mg just wasn’t cutting it. I was then put on norethindrone 10mg/day in April 2011. I had occasional episodes where I would wake up in the middle of the night in horrific pain and have difficulty moving and speaking. I got cleared by neurosurgery just to make sure these episodes weren’t neurological and I was safe to have surgery, and we proceeded with my second surgery on July 11, 2012. During this surgery, we found my endometriosis progressed to stage II (anterior/posterior cul-de-sacs, both pelvic side walls), so Dr. Laufer said this hormonal treatment clearly wasn’t cutting it and that I wasn’t supposed to be progressing while on hormone therapy. But I was told I was lucky since it appeared to me that my pain got significantly worse with progression, so I probably wasn’t going to end up with a ton of internal damage without anybody knowing about it.

Both egg and ovarian tissue cryopreservation come with their own merits and demerits. Ovarian tissue cryopreservation, while not available in most settings, is an effective technique for fertility preservation, especially where the laparoscopic removal of endometriomas is necessary. Here, the surgeon isolates and preserves the healthy parts of the ovarian cortex containing the follicles. They can then transplant them back into the body at a later date. Another advantage is that there is no need for stimulation as with egg freezing and your surgeon can do this during laparoscopic surgery itself.

We follow a more hands-on approach in order to properly differentiate between scars, adhesions, and lesions while navigating inside the body. This is simply not possible with robotic-assisted laparoscopic techniques since the surgeon has no tactile feeling of the tissue and must rely solely on video information. Also, the incisions made by robotic assistance are larger compared to those made manually by experienced surgeons.

Physical Therapy
Neuropathic pain occurs when there is damage or compression to a specific nerve. Unlike the sensation of aching in nociceptive pain, neuropathic pain is in the form of tingling, burning, and shocks that can radiate to areas away from the site of nerve damage. NSAIDs do not reduce neuropathic pain and the use of drugs like gabapentin is necessary. Sometimes, anesthesia, physical therapy, or even surgery may be necessary.
